Transition plan template: Year 6 to Year 7
A transition plan is a hand-over for the pupils most at risk at a move, especially Year 6 to Year 7. It is started before the change: extra visits at quiet times, meeting the key adult, a map of the building and a record of what helps. The secondary version adds an exam-season planner.

How to use it
Start it in the summer term of Year 6, not in October of Year 7. The aim is to make the new building familiar before it becomes frightening.
Flag any earlier separation difficulty or school avoidance, even if it was mild. It is the single most useful thing a primary school can pass on.
