EBSA early-warning checklist
The EBSA early-warning checklist logs the shape of a pupil’s absence, not just the total: Monday patterns, lateness, single missed lessons, school-morning stomach aches that vanish at weekends, and the pupil who copes all day and falls apart at home. Start it at around 94 per cent attendance, well before persistent absence.

How to use it
Fill it in as you go rather than at the end of term. It is the pattern across several lines, not one tick, that tells you something is wrong.
If the picture builds, open the first conversation curious rather than concerned, bring parents in early and without blame, and share the sheet with your SENCo. Any worry about safety or welfare goes to your DSL the same day.
Background: What is emotionally based school avoidance (EBSA)?
What’s on it · Primary (Reception to Year 6)
Keep one running for any pupil who's started to niggle at you, fill it in as you go rather than from memory at term's end, and start watching from the 94% mark — not from persistent absence.
Pupil: ……………… Year: …… Key adult: ……………… Date opened: ……… Current attendance: ……… % (watch from 94%)
Tick what you're seeing and add the detail — it's the pattern, not any single line, that tells the story:
The 94% line crossed? ☐ Yes → open the first conversation curious, not concerned, bring parents in early and no-blame, and share with the SENCo. ☐ Not yet → keep logging.
What’s on it · Secondary (Years 7 to 11)
